298 Improving AI Assessment of Cutaneous Chronic Graft-Versus-Host Disease using Unlabeled Patient Photographs

OBJECTIVES/GOALS: Measuring the area of skin involvement in chronic graft-versus-host disease (cGVHD) relies on costly, time-consuming manual assessment, with high disagreement among experts (>20%). Our published AI method, trained on labeled 3D photos, showed promise for delineating affected areas. We aim to improve its performance using unlabeled 2D photos. METHODS/STUDY POPULATION: Our published AI model (baseline) was trained on 360 labeled photos of 36 cGVHD patients,from a 3D camera with calibrated distance and lighting.Our gold standard labels were contours around affected skin, marked by a trained expert. A second unlabeledcohort of 974 standard 2D photos of 8 cGVHD patients was used to improve the baseline model. First the baseline model predicted affected areas on the unlabeled photos. Photos with good predictions were added to the training set with their AI-predicted labels. The model was then re-trained with the expanded labeled set. Models were successively trained with more AI labels until performance stopped improving. AI performance was assessed on a test set of 20 photos from 20 patients unseen during training, labeled by 4 experts to improve accuracy. RESULTS/ANTICIPATED RESULTS: Model performance was calculated by comparing against the gold standard labels on the test set. To quantify the spatial overlap of labeled areas the Dice coefficient was used (0 is no overlap, 1 is complete agreement), where higher values are better. To estimate clinical error we used surface area error (Error), where lower values are better. On the test set, the baseline model had a median Dice of 0.57 [interquartile range: 0.39 – 0.82] and Error of 57.6% [20.2 – 103.3%]. Re-training with additional AI-predicted labels from 8 new patients, the model yielded a median Dice of 0.60 [0.35 – 0.80] and Error of 50% [12.5 – 103.8%]. This approach is being expanded to a further 300 unlabeled patients, where we anticipate significant improvements to AI performance and consistency. DISCUSSION/SIGNIFICANCE: Evaluating AI models in standard photos could provide a consistent method of assessing and tracking cutaneous cGVHD and relieve the burden of costly expert assessment. A reliable automated AI tool would provide a meaningful improvement to the current standard of manual assessment and could be easily applied to large patient cohorts.

Synthetic auxin herbicides were developed and commercialized 60 yr before their mode of action was definitively elucidated. Although evolution of resistance to auxinic herbicides proceeded more slowly than for some other herbicide chemistries, it has become a major problem in the dicotyledonous weeds of many cropping areas of the world. With the molecular characterization of the auxin perception and signaling pathway in the mid-2000s came a greater understanding of how auxinic herbicides work, and how resistance may develop in weeds subjected to repeated selection with these herbicides. In wild radish (Raphanus raphanistrum L.) populations in southern Australia, resistance to multiple herbicides, including synthetic auxins such as 2,4-D, has reduced the number of chemical control options available. The aim of this study was to determine whether compounds involved in auxin biosynthesis, transport, and signaling are able to synergize with 2,4-D and increase its ability to control 2,4-D–resistant R. raphanistrum populations. Although some mild synergism was observed with a few compounds (abscisic acid, cyclanilide, tryptamine), the response was not large or consistent enough to warrant further study. Similarly, alternative auxinic herbicides applied pre- or postemergence were no more effective than 2,4-D. Therefore, while use of auxinic herbicides continues to increase due to the adoption of transgenic resistant crops, nonchemical control techniques will become more important, and chemical control of 2,4-D–resistant R. raphanistrum should be undertaken with alternative modes of action, using mixtures and good stewardship to delay the development of resistance for as long as possible.
Exploiting high-slip flow regimes to improve inference of glacier bed topography

Theory and observation show that glacier-flow regimes characterized by high basal slip enhance the projection of topographic detail to the surface, motivating this investigation into the efficacy of using glacier surges to improve bed estimation. Here we adapt a Bayesian inversion scheme and apply it to real and synthetic data as a proof of concept. Synthetic tests show a reduction in mean RMSE between true and inferred beds by more than half, and an increase in the mean correlation coefficient of ~0.5, when data from slip- versus deformation-dominated regimes are used. Multi-epoch inversions, which partition slip- and deformation-dominated regimes, are shown to outperform inversions that average over these flow regimes thereby squandering information. Tests with real data from a surging glacier in Yukon, Canada, corroborate these results, while highlighting the challenges of limited or inconsistent data. With the growing torrent of satellite-based observations, fast-flow events such as glacier surges offer potential to improve bed estimation for some of the world's most dynamic glaciers.

Local communities and their schools remain key sites for actions tackling issues of sustainability and climate change. A government-funded environmental education initiative, the Australian Sustainable Schools Initiative (AuSSI), working together with state based Sustainable Schools Programs (SSP), has the ability to support the development of more effective community and school relationships. We are interested in the possibilities of enabling more authentic and transformative learning experiences in community and school relationships, by developing a more analytical approach to communities and schools working together. Drawing on Uzzell's (1999) framework and a number of recent empirical studies we describe how communities and schools in one Australian State, New South Wales, have been working together for environmental sustainability. We point to how the links between local communities and schools continue to be under-utilised, and suggest ways that these important relationships can be strengthened and extended.

Aim
To explore GPs’ experiences of managing recurrent urinary tract infections (RUTIs) and their views on the use of herbal medicines for this condition.
BackgroundRUTIs are an important problem commonly managed in primary care. Antibiotic prophylaxis is an effective treatment for acute infections but growing microbial resistance, adverse effects, and the lack of sustained long-term benefits mean that novel treatments are required. There are a number of promising reports of herbal medicines being used to treat RUTIs.
MethodsA total of 15 GPs (seven female; aged 34–59 years; in practice from 3 to 31 years) were purposively sampled and took part in semi-structured face-to-face and telephone interviews. Interviews were digitally recorded, transcribed, and analysed using inductive thematic analysis. Data collection and analysis proceeded iteratively to allow emerging themes to inform subsequent interviews.
FindingsParticipants were aware of the disabling effect of RUTIs on women’s lives. GPs experienced significant challenges in their management of RUTIs with decisions about the provision of antibiotics being particularly complex. While some participants were open to the possibility of herbal treatment options they required more research into effectiveness and safety, better regulation of herbal practitioners, and assurance about herbal quality control and potential herb–drug interactions.
